How Wishes Are Granted - Wish Referral Form

Who can refer a child for a wish?

Out of respect for the privacy of the children and families we serve, children who may be eligible to receive a wish can be referred by one of three sources:

  • Medical professionals treating the child - such as doctors, nurses, social workers or child-life specialists
  • A parent or legal guardian of the potential wish child
  • The potential wish child

Who is eligible for a wish?

Any child between the ages of 2.5 and 18 who has been diagnosed with a life-threatening medical condition, i.e. a progressive, degenerative or malignant medical condition that places their life in jeopardy.
